Library Instruction Request Form

Instructional Services Librarians are eager to assist faculty by teaching them or their students how to best use library facilities and materials. Please use this form to request a library instruction class.  If your browser does not support forms, you may print this form and send it to Jane Goodman at the University Library, or call her at 381-3331. Instruction classrooms are located on the south side of the first floor of the West Wing of the Library. Classroom #1 (1.122) has 34 computer workstations; classroom #2 (1.123) has 23 workstations. Both classrooms have a white board, projector and screen, and multi-purpose instructor's workstation.  Please request your class at least one week in advance; the deadline for requests is noon Thursday of the week before class. We will make every effort to schedule valid class requests, but sometimes there are scheduling conflicts.  You will be contacted to reschedule if necessary.  If you submit this form, leave us a voice mail, or email us, and do not hear from one of us within a couple of days, please try to contact us again.
